COORDINATOR, SEXUAL HEALTH ADVOCATES

Contractual Obligations
1. Implement evidence-based intervention (EBI) services for LGBTQIA youth, focused on effective, youth-based sexual health education and risk-reduction programming
a. Recruit, train, engage and coordinate eight (8) peer leaders from HMI youth community
b. Provide peer leaders with comprehensive and ongoing training related to sexual health & HIV & STI prevention, as well as effective leadership skills to be peer mentors
c. Develop and implement activities that actively engage youth to seek status-neutral sexual health services using social networking strategies and peer mentorship models
2. Conduct and assist with peer-led community-level sexual health workshops
a. Identify and secure community-based organizations, school, community centers and other partners to implement The Alexis Project EBI & community activations
b. Conduct two cycles/year of an adapted version of CDC's Alexis Project intervention, to include up to 4 peer-based youth interns per cycle and focused on LGBTQIA+ young women and girls of color
c. Recruit and engage participants throughout NYC to participate in community activations
d. Community mobilization and the dissemination of peer-led sexual stigma reducing and HIV prevention visual and performative campaign through in-person and social media engagement
e. Maintain appropriate data systems to report attendance, evaluation and outcomes
3. Develop and implement evaluation tools and resulting data to inform program creation and design
4. Work in conjunction with HIV Services team of navigation specialist to perform outreach, screening, and assessment of sexual health needs among HMI members to ensure appropriate referrals to HIV/STI and other gender-affirming services internally and throughout the community.
b. Outreach and engage youth to engage in navigation support using need assessment and screening tools
e. Refer to HIV Services team to develop support plan
f. Directly link participants to sexual and gender-affirming health care and other needed services (e.g., dental, mental health, substance abuse treatment, job training, legal)
g. Send appointment reminders and provide transportation to appointment(s) if needed
h. Have ongoing contact with participants, especially after referral and linkage to services
5. Assist with referrals for improved access to biomedical prevention services for persons at risk for HIV, including:
a. Referral to PrEP/PEP/Doxy-PEP education and care services
b. Linkage navigation to clinical site providing PrEP/PEP/Docy-PEP and other services
6. Support with coordination and scheduling regular HIV/STI testing events (on-site and community-based), when possible, as well as development of monthly HIV/STI testing and screening campaigns
7. Facilitate participation in on-site gender-affirming sexual health support services
a. Support with 1:1 gender-affirming youth sessions to provide members with needs assessment and select personal items, such as binders, gaffs, T-tape, education, and other support
b. Support with scheduling and facilitating partnering medical providers (e.g., HOTT Van Services and vision care) for youth; Identify and secure dates of partnering provider service provision
c. Locate and mobilize community resources including community referrals to support youth and families.
Maintain client-level data, including follow-up notes and referral logs.

Work closely with other HMI departments and staff to support ongoing programs and services.
a. Outreach and engage youth during Program Common Hours (3:30pm - 7:30pm)
b. Prepare for and facilitate weekly group-based activities for general programming, when needed
c. Assist in the facilitation of Health & Wellness groups and programming
d. Perform general staff duties, such as Café duty, general event set-up/breakdown, etc
e. Participate and assist with general HMI agency-wide events
Administrative Duties
25%
Track youth member enrollment/activities and contractual requirements, and prepare reports and paperwork requested as needed by agency or funders
a. Maintain client level data, including follow-up notes and referral logs
b. Maintain group level documentation, including sign-in sheets and session summaries
c. Document, code and enter data for program analysis
d. Submit all necessary documentation to Data Coordinator(s)
